Not to me, but I attended an Equality & Diversity training course (in the UK), and apparently it is! In this PC mad world we live in, almost every word is now considered offensive. It really is ridiculous.
An example we were given during the course was the phrase "You're such an old lady!". Apparently, the correct word to use is "woman". That to me sounds worse, but then I'm not a politically correct person.

No, not to me (and I'm a lady). It's not offensive; it's just not formal. But some politically correct, liberal wackos would argue that it is offensive and say you should use the word "lunchperson" instead of "lunchlady"... but lady is fine with me.

Depends who you're calling a 'lady' & in what context I guess.
but why would it be offensive in the polite form? There are lots of different ways of using it, such as 'Lady' of the night'. Or Little Btitain's 'I'm a laydee'. Or perhaps in northern England 'Now listen, lady'.
So probably depends on who's saying it to whom & under what circumstances.
I'm female & I like to be treated or called a lady sometimes. Can I please get on with this bloody cream cake, now?LOL!

If you say it to a man perhaps, but otherwise no. If anything it is a respectful term - more polite than saying woman.
