well there are accidents. the twilight zone incident is the worst that comes to mind. In which an actor and two children were killed during a stunt. its rare in U.S for these accidents since the insurance company and U.S. Government require major safety precautions. Outside of the U.S. they laws are more relaxed, especially when it comes to stunt work.
As for films with portrayed violence no they don't get killed. Most deaths occur during stunt work. Car chases, explosions, and fight scenes. Movies like Saw,hostel or Turista are done with mostly makeup effects, or props and these don't commonly make for accidents.

Actually Brandon Lee was killed durring the 1st Crow in 1994.
No that doesn't happen. There have been acidental deaths (such as Brandon Lee) in the process of making movies and I am sure the insurance company paid a hefty price tag but no there is no mass killing that are real in the movies. How would they get away with that?

Victor Morrow, Rob Morrow's father and 2 children were killed during a filming sequence for Twilight Zone the movie. In the sequence which was supposed to be some sort of flashback from Vietnam, a helicopter was flying near them with explosions going off around them. One of the explosions went off too close to the helicopter causing it to roll sideways. At the time it was flying low to the ground near the victims. The blades from the helicopter actually hit the ground and them as well. Gruesome, horrible, tragic and very sad.

During the filming of the famous chariot race in 'Ben Hur', one of the extras who was on a chariot fell down into the traces, and was dragged along and trampled to death by the horses.
It was an extremely costly, time-consuming and difficult sequence, so the producers were very relieved when the dead actor's widow agreed they could use the take in the film.
So, if you ever see that film which is a terrific movie by the way, when you watch the chariot race, you do actually see a man die.

Check the story on the making of The Twilight Zone with Vic Morrow.
Also in the making of Ben Hur a stunt chariot driver was trampled and killed when his chariot overturned during the race scene. The widow insisted it be left in the film and it was.

The only ones I can think of is the father then son...Bruce Lee, (November 27, 1940 – July 20, 1973, mysteriously died) and his son, Brandon, (Feb-1965, d. 31-Mar-1993, accidental gunshot) Whom were famous Martial Artists...accidentally, Tragic.
But otherwise I wouldn't think there hasn't been any others....I mean afterall>>" You're at the 'Movies'"!....

No, they do not get killed. It's a movie/T.V. show. It's fake. I really don't think someone would waste their life, like that. There is such a thing as fake blood, and all that, like fake cuts and heads, and all that. Of course, the producers of the movie or show, go to a professional store, not Halloween stores, or stuff like that!
