I haven't had a raise for over a year and fell that I have taken on more responsibilities and have excelled with those duties. I would like to present my boss with the accomplishments and ask for a raise. How should I do this

Exactly as you outlined in your question - gather your accomplishments, schedule some time with your boss that is less likely to be stressed (not first thing on Monday or last thing on Friday, for example), and then say something like, "Boss, i've been here for a year and I wanted to sit down with you and discuss my performance. I feel I've contributed in area a, project b, and so on. I'd like to be considered for an increase in my compensation. Can we discuss how that might be structured?"

You aren't saying, "can I have a raise?" because the answer could then be, "no, you can't." You're saying, "how do we adjust my comp to a reasonable level?"
