i was in the hospital and out of work for two weeks... i need to know what kind of paperwork i need to fill out and where do i get this paperwork.i went to the hospital from work but it was not work related... i work 40+ hours a week.

Not knowing what state you are in makes this difficult to answer. There are only a few states that have state disability plans. (NJ, NY, CA, HI and RI) If you are in one of these states, your employer should have the required form that will need to be completed.

If you are not in one of these states, your employer may have private disability coverage and again will be able to give you the paperwork, otherwise, since disability coverage is not required, than you do not have coverage for disability

Since it was not work related, the hospitalization would be paid by your health insurance. You should be able to get claim forms from your HR Department. The time off would be under your state unemployment department, if you don't receive paid time off from your employer.
