i work full time and i was just put in the hospital and i was out of work for two weeks? i was at work when i went to the hospital but it was not work related...also i don't get any sick days at work.need to know where do i get the paperwork.

The best thing for you to do is contact your company's Human Resource Department and inquire there.

Not knowing what state you are in makes this difficult to answer. There are only a few states that have state disability plans. (NJ, NY, CA, HI and RI) If you are in one of these states, your employer should have the required form that will need to be completed.

If you are not in one of these states, your employer may have private disability coverage and again will be able to give you the paperwork, otherwise, since disability coverage is not required, than you do not have coverage for disability

Paper work for what?

If you don't get sick days then you don't get paid when you're sick. Why should they pay you for days you didn't work?
