my friend has just asked me this out of the blue on messenger should i be worried?
its not like her. i have known her over ten years. she has also mentioned HIV and cancer

You have a responsibility to pursue this one a bit. Lies can be vicious and have been known to wreck lives of innocent people. I'm not saying this is what is happening, but it does sound possible. If you've known her ten years, this should be a straight forward conversation...no cat and mouse game.
