I am 42 and at the very top of the pay scale for my industry (PHP programming). I'd like to get more money, but a raise or a new position in the same industry seems unlikely. I'm not much of a self-starter so starting my own business would not be viable.

So cross-train in my current career? Or switch careers?

I'm not unhappy doing what I am doing, I just know that I can't really move UP in the work I'm doing. I am just looking for advice about moving forward in planning my future.

If you are unhappy with your current situations there is no use in just staying put and being miserable.

Talk to your supervisor maybe you can take on more responsibilites or maybe they will know of another opening in another department that you would be a good canidate for and you would enjoy.

The answer is simple: follow your heart and do what you've always wanted to do.
