In California, if you go on pregnancy disability leave does your employer have to continue your medical coverage?

Yes but not for free and since you're not at work earning a paycheck that they can take your contribution out of..you'll probably owe money when you come back.

I came back five months ago and my entire first paycheck when to reimbursing the company for my medical insurance contribution.

Speak to your HR department, they ought to be able to inform you of your coverage and your rights.

Don't forget not only do you get to go on state disability for 6 weeks (vaginal) or 8 weeks (csection) but you also have access to Paid Family Leave for an additional 6 weeks.

Your medical will continue but usually at your own expense. I ended up having to pay over $400/month for my medical while I was on maternity leave.

Yes, because you're still employed with the company and you're on granted leave.
