Everytime I hear a little sound when I'm sleeping, I let out a scream! Iv'e always had nightmares since I was 13! I wake up screaming, and sometimes have gotten out of bed , and start to run, and usually fall down and scrap my knees! They seem to get worse when I'm stressed or unhappy, or bothered about something!

These are called Night Terrors and will usually fade away as you get older. You already know that if you are worried about things, you experience this, so this is anxiety related. Anything relating to seperation anxiety is a biggie with this and will cause an episode. Finding some form of comfort to sleep with will help; a stuffed animal, a blanket, or a piece of clothing clutched or worn while sleeping that brings some source of strength for you. Another cause may be fever from an oncoming illness and certain cold and allergy medicines can bring on a Night Terror episode, so can being overtired.
If this persists, you may wish to contact someone from a Center For Sleep Disorders near you for additional advise & help. My group's link is below if you have any further questions. Good luck & Sweet dreams! :)

Usually it's because you have a fear of something in real life. Try confronting that fear or eliminating it and you should stop waking up like that. When you sleep, you don't actually dream until much later in the night, like 4 or 5 p.m. So whatever is bothering you must be occuring pretty late during the night. Make sure nothing wakes you while you sleep. ie: alarms, tv's
