(Another stupid one is "gone missing". A 98 year old man was discovered gone missing from his Florida townhouse this morning...)

I notice a lot of kids when first learning to talk say this when they've done something wrong. I suppose people just thought it was cute and decided to use it. I see it a lot in tv now.

I first heard it from a bunch of slacker 20-somethings who hang with my boss' son. All 20-somethings seem to say it (though not all are slackers, lol) Possibly a movie they all saw...
