Any such work as a book title, poem or song should be capitalized.

Are you talking about a specific poem or just in general? You follow the general capitalization rule for all titles unless it's a poet who is known for not using capitals, like e.e. cummings. Then you print the title just as it appears on the published page.

yes unless you are e e cummings who did not capitalize anything. If you are writing about a poem you would also put the title in quotation marks.

You should capitalize the first letter of each word of any title. (Except words like "and", "or", etc.)
