I haven't read or thought about that poem for many years.
Being a slave meant suffering treatment that was sub-human. Yet all around her were "ladies", white women of standing, dressed nice, and treated like delicate personas deserving of preference and service. So she points out, "ain't I a woman?", to draw attention to the fact that color doesn't make her any less than the females around her getting preferential treatment. In fact, she deserved more than the average white woman of her time.

The work which black women have been forced to perform, either in slavery or in a discriminatory work place, that would be non-gender conforming for white women has been used against black women as a proof of their emasculating behaviour.
