Because Pilates targets the tummy and pelvic floor muscles and these muscles can weaken during pregnancy, Pilates exercises can be useful. Many Pilates exercies are performed on a "hands and knees" position, and this is an ideal position for pregnancy. It helps to take a lot of stress off your back and pelvis and towards the end of your pregnancy can help to position your baby ready for delivery.

The page below I think you will find very useful.

I would opt for a pregnancy DVD. There are some stretches in Pilates that can cause you problems due to the somewhat awkward changes in your body.

There is a safe way to do Pilates during pregnancy. See http://pilates.about.com/od/pilatesforeverybody/a/pregnancyBasics.htm

Pilates focuses on core muscles. It helps to strengthen your back muscle for child bearing. But it might be too strenuous for a 4mths pregnant lady. Try Pre-natal Yoga! Incorporating props, like cushion and chairs. it is more mild and safe.
