A clapperboard, it's used to cue the camera and microphones.

The clapboard is used to sync sound to picture.

The scene 4 take 5 identifies the sound clip.

The writing on the slate identifies the film clip.

Sync the two together by matching the clap to the frame where the black & white bars on the clapper are in focus (that means it stopped moving and thus the clap sound).

Directors sometimes like to take more than one shot of a scene.Sometimes it's to examine another angle or to simply re-shoot to cover a mift line by actors. The clapboard is used to log each scene for editing purposes.
