If it is a form for benefits, you tell the truth. Lying will get you caught (benefit providers check social security numbers, for one thing) and you may end up having to pay back the cost of the benefits.

You can always say something to your boss like, "well, we think of each other as husband and wife, so that's what I call him/her. We just haven't managed to make it legal yet."

Tell the truth on the form. I sure hope you haven't lied to your boss or lied on any other employee forms. If so, you may end up fired, depending on which forms you're referring to.

Legally your boss can not hold anyhting againist you if ur married or not its none of their business but you should just be honest.
