Well, if you are talking about the Bloody Mary in a child’s game, where you go into a bathroom, and do turn around, and chant out ‘Bloody Mary’, and wait for this evil ghost to come through the mirror, then she is not true.
However, in history there was a Queen Mary I of England. She was into her Mother’s (Catherine of Aragon) religious faith, Catholic. Since her Father had made the Anglican Church which was Protestant, Roman Catholics lived in fear, because they were hated by the Protestant. When she married her husband, a roman catholic, Philip II of Spain, they began to do a reformation, by killing all the Protestant. Thus, with so many killings, her nickname became Bloody Mary.
So, yes in history there was an actual Bloody Mary.
I hope this helped.

Bloody Mary is a name applied from the british to both Queen Mary I of England. Mary I acquired the nickname during the reign of her successor Elizabeth on account of the perceived ferocity of her religious persecutions.Queen Mary I of England (reigned 1553 – 1558) is called Bloody Mary because of her persecution of Protestants. The Protestant Reformation, started fitfully in England by King Henry VIII, became established policy under the brief reign of Edward VI. Mary, like her mother Catherine of Aragon, whom Henry VIII had spurned, was a devout Roman Catholic. She contracted an unpopular marriage to the equally devout Philip II of Spain. The pair, with the assistance of conservative churchmen such as Edward "Bloody" Bonner and Stephen Gardiner, sought to reimpose Roman Catholicism upon the religiously divided country.

yes and no, Bloody Mary is a name applied from the british to both Queen Mary I of England and to a figure of Western folklore. Mary I acquired the epithet during the reign of her successor Elizabeth on account of the perceived ferocity of her religious persecutions. The folkloric character is a ghost or witch said to rape, kill, squeeze, eat, and bury children; other very similar tales use different names. Several other fictional characters are also called Bloody Mary.

Queen Mary of the Scotts. Every year she put out a death warrant to every protestant in scottland. Many were killed for no apperant reason other than she wasnt a protestant. When she was executed, the executioner chopped off half of her neck then stepped on her back to get the axe out of her neck. When he was finished he got executed in the same manner for stepping on the royal back. Her last words were, "may my death be swift"
