You're worried WAY too early. You need to learn how to have a balanced life.
To ease your concerns...Get good grades MOST IMPORTANTLY in your math and English courses. Get AT LEAST A 1200 (or even 1300) on your SAT's.
You also MUST do extra-curricular activities. This means a sports team, a club, student government...This is VERY IMPORTANT when it comes to college admissions because they think you're a "well rounded" person.
You must be careful though NOT to let these curricular activities take TOO MUCH time from your studies. They come first.
DESPITE ALL THIS....YOU NEED TO RELAX. You're only young once. STop worrying.
The fact that you're already thinking about college tells me you'll do JUST FINE, and get into any college of your choosing. So relax.

First of all, calm down. You are only in 7th grade- you have plenty of time. In most school districts, 8th graders who get A's in math, science and english can take geometry and biology freshman year. This gives you a good advantage if you can start highschool this way. If you want to be a plastic surgeon, your focus should be science and math, so definately stay on top of those grades and try to reach you highest level. If your highschool offers AP math and science classes, these would be good classes to take to get into a good med school. Just know that being a plastic surgeon takes a minimum of 8 years of college- undergraduate and then grad school. Besides taking challenging classes, volunteer or get an internship at your local hospital or clinic. Colleges like to see extracurriculars, so this would be good to do. Good luck!
Also: (If you are good at a sport, you may want to work hard at it, because if you are good enough, you can get a scholarship or get accepted to a good school based on your athletic ability.)

Generally, colleges don't start looking at transcripts until high school starts. To be appealing to colleges make sure your grades are decent, you do extracurriculars, volunteer, and try to do well on the SAT or ACT. Don't worry to much, have fun!! High School is one of the best times of your life, enjoy it.

take algebra in 8th grade and french and as many honors classes in math and science that you can take and handle.
9grade: AP geometry, AP Bio, AP/honors French, Ap?honors world history, AP Literature/ AP art history, and so on
Do well on the ap exams get 4 or 5's then before your senior year take ap calculus ab and ba and get a 4 or 5 on the ap exam go to college that is Recognized like NYU, UChicago, BC BU, Northwestern. Get a 3.5GPA do well on the MCAT do academic reasearch with a top internationaly recognize chem or bio professor with is pubished and writtien books that is on the NY times best seller lists and barns and noble top seller list and is been on tv meet the press and so on. Then hope.
Law school is better and a bit easer to getinto .

Take advanced classes and get in to different sports if you like sports and when your a senior get into the NHS national honors society and get in band (if you want) sports and band will help with scholorships to your college you choose and it will make it more affordable b/c what you want to be is ganna cost losts of money!!! Always be the first with your hand up and get the teachers to like you!!! If you put them as references on your college application they will tell how good you are at school and stuff!!!

Do some volunteer and internship work in your field of interest. That really helps! Also, do well to prepare for standardized tests and keep your grades up.
