Good question.
It's only selfish if that's all you do. There needs to be a healthy balance of when you put yourself first and when you put others first.
If you go to the bathroom before feeding the kids, that's not selfish, that is making sure you can give them better attention. They won't starve to death in the next two minutes.
If you go to the bathroom before tending to your kid's bloody knees after a fall, that is selfish. You can stand to hold it in for a couple of minutes to make sure the wounds get covered and do not get infected.

People who accuse others of selfishness because they take care of themselves first at appropriate moments are selfish themselves.

This is a difficult question. I guess it depends on what you mean by "taking care of yourself." If that means going out and staying out late neglecting home duties, then that seems selfish. Or buying yourself the nicest things and buying your family cheap things, then that seems selfish.

From a psychological perspective, I think it is important to realize if you have issues that may affect your ability to be in a relationship or take care of your children, if you have any. That is not selfish. Hopefully it means that you are trying to work on them so that you can be the best person you can be, knowing nobody is perfect.

If you take care of yourself first with the intent to serve God and others. Then you are not selfish. Selflessness is to think of your self less. So It really depends on one inner intention. Read Bhagavad Gita As it is By Bhaktivedanta Prabhupada -tells how to actually be selfless and yet love ones self fully.

I don't know who gave you the idea that Your needs and Your health come first, but they led you astray. When you find a way to help someone weaker than you are or has less than you have, then you will gain your own self-respect by lifting them up higher and no one can ever take that away from you. You Do Not Come First in this world. You were misinformed.
