what is the recipie for making the sodium acetaite found inside the reusable hand warmers

CH3–COOH + Na+[HCO3]– → CH3–COO– Na+ + H2O + CO2
It is the reaction we all know and love, vinegar + sodium bicarbonate. Then if you boil off most of the liquid you can create a concentrated solution. I'm a little confused to how the reaction starts inside the hand warmer when you want it to though (try the two links below).
