She is technically his cousin. Nejis father, has a brother, who has a daughter (Hinata) Neji actually has two cousins if you havent noticed yet (Hanabi). She is CALLED his sister sometimes because thats how the hyuuga clan goes. Neji was born to protect Hinata, being her older brother almost like a guardian.

Cousin, she just called him 'Neji-niisan' cuz dats how some Japanese address a person who's a few years older than them. It's not limited to being blood related.

yup their cousins, but hinata is from the main branch of the hyuga clan, while neji is from a side branch who is obligated to protect the main branch
