Do you mean that your ISP required a particular MAC address so your router has to spoof the MAC address of your computer? If so, Start > Run and type
cmd
and hit enter, then type
ipconfig /all
and one of the items is the physical address of your network card.

Start>Accessories>System Tools>System Information

Open up Components>Network>Adapter

To the right, in the list, it'll show your MAC address.

For your MAC address, just run "ifconfig -a"

For your hostname, simply run "hostname"
