A complete betrayal. Not a misunderstanding or accident, but your friend doing something deliberately to hurt you. But even then, in a great friendship, that person may forgive.

i'll tell u what damaged the best friendship i ever had
he met new people and pushed me aside slowly. e neglected me saying he though what we had was strong and that i understood why he had me take the back seat. so i told him it wasn;t right and he agreed. bbut he continued the same thing. and then he didnt visit me in the hospital. then i told him no more. and that was final.he didnt try to stop the realtionship from cesing.

There are many causes, just like there are many causes to start friendships. I can't list them all here, but I guess betraying your friend's trust and ignoring them is a biggie.
