I think it depends on where they work, if they're in a hospital, they would wear scrubs, and if they are in a doctors office, they wear normal clothes with a white doctors coat. Google pediatricians work attire.

When my children were younger, their pediatrician wore a white coat over his dress shirt, & worse dress slacks.

That can vary quite a bit based on the doctor's personality, the facility they're working in/for and what they're doing that day.
