Chimney boy to sweep if it's old.

Most of the ones I've seen were for cleaning purposes.

My chimney sweep used to keep his vacuum outside and run the hose in through that little door. That way if the filter went bad or something, it would be venting outside.

When the door is down at the back base, it is to sweep ashes out.

I call it a flue, some call it a damper, but what it is used for is to close off your chimney in the summer and open it when winter comes if you are building fires.

You can test this: Close it and start a fire in the fireplace. Smoke will fill your house. Open it and start a fire in the fireplace, and the smoke will rise up the chimney.

You said "up," so I'm guessing its the "smoke shelf." it keeps out rain and debris and can be closed when your not using your fireplace. Could probably help con troll the air-flow and consequently the intensity of the fire, if its adjustable, also. If you're talking about the door on the bottom...That's just the ash pit.

When closed, the heat of the coal fire comes out into the room. When open, it is used to heat the water tank.

That's the damper, it's to keep the cold air out along with rodents when the fireplace is not in use or to regulate to rate of exhaust when it is in use, usually you keep it full open.
