What kind of information do you exactly get on this form? I am looking for my state of residence, will that show up?

A 4506T is a form you fill in and sign to allow a third party to have access to your income tax records on file at the Internal Revenue Service.

You fill in your own name, address, social security number etc. Don't you know your state of residency?
