Yes, where I am from you do. And yearly after that to get a new prescription for birth control married, single, virgin does not matter.

99% of the time yes. If you are sexually active you need to really have one every year anyway for your own health.

Well where I am from you dont have to. You dont have to get one untill you are 18. It use to be your first period or first intercourse. So it may be different but around here no you dont have to!
