a new ball is used at the start of every innings in both 1 dayers and test matches. a white ball is used in 1 dayers and towards the end of the innings sometimes the ball gets to discoloured and it becomes hard for the batters to see, so its up to umpire's discretion to change the ball for 1 that is easier to see but a new ball wont be used, they have a selection of balls that have been used before for roughly the same amount of overs, but that still have enough colour on them for the batter to see.

Yes a new ball is used in every innings of a match. However, in test matches a new ball can be taken by the fielding side after 90 overs .

yea we do especially in one day cricket beginning of every innings a new ball is used in tests it is something else if the team that was bowling just took a new ball then when the other team comes out to bowl they will have to use the same ball
