I am 21 year old female and I dont consider myself an expert on sex or anything of that sort but I went to borrow some paper from my 18 year old brothers tablet and I saw a paper he had on the inside front cover and it was a list of women he " spanked the monkey to " and I couldnt believe all the women on there. It was like pages of 500 plus women from celebrities like Lindsay Lohan to some of my parents female friends to even female cousins. There was also a section called " skirt with no panties section " with names underneath that. Should I be alarmed? Should I show this to his school counselor or tell my dad? I am worried.

I don't think you really haven anything to worry about. He and his friends were probably trying to "one up" one another in their he man stories and he just made a list or has fantasized about them. If you are really worried then I would confront him rather than your dad. The counselor might be able to just have a talk with him to make sure he is okay but as I am the mother of two tenage boys. . . . .I have over heard them talking and they talk the talk but I KNOW that my boys are all talk because I keep a very tight leash on them but if they want to brag about some fantasy or something like that I think it is pretty normal for guys. They have very active hormones at a much earlier age than girls do. Unfortunately that is what they think with rather than their larger head until they are about 25 and even then they have relapses.

Do nothing. HE IS just dreaming on paper. Most teen age girls have them too, yet not so graphic I would hope.
Let it go. If there were indications of harm, anger, abuse or anything alarming like that, then point it out to DAD.
Never pull an outsider into your family unless you have no other choice. Today's world is too judgemental and it would really hurt the entire family unit.
If you MUST do something, because its about you, then talk to your parents and list it as a concern.

I don't think that you should say anything to anybody about this, but your brother. It is none of your dad's business or your school counselor business. Your brother is 18, and he can do what he wants. If you are alarmed by this then you should confront your brother and ask him about it. Tell him you are worried about him sleeping with all of these women, and that you hope he is using protection b/c he could get something. Just tell him you love him, you worry about him, and that you hope he is being safe about this. Apparently your brother LOVES the ladies, so just let him have his fun. Theres nothing wrong with that!!

9 out of 10 guys admit to whacking off and the other 10% lie about it. ok. Making a list is a little odd but not anything to be worried about. Just be glad you are not on the list.

Just do your best to remove the picture from your head , he just has raging hormones and it's basically harmless though understandably disturbing for you to have seen it . It wouldn't serve a use full purpose to bring it up other than humiliating your brother .
