The code "12" is an indication that there are no codes stored in the PCM, and that the PCM is communicating properly. The code "12" is an identifier that there is no ignition pulse. If the engine isn't running, there wouldn't be a pulse. All is well there.
As for the hard starting, the most likely cause is that the fuel pump may be on the verge of failure.
Most 4.3s of this vintage have TBI injection unless they are "VORTEC".
If the fuel pump anti-drain valve is failing, then the fuel will drain from the system, and will need to be "primed" again. When you hit it with ether, the same effect takes place.
Try This:
The next time you need to start it when it will be difficult, turn the key on for 2 seconds (don't try to start it, just turn the key to "on"), then turn it off for 5 seconds, then on for 2, then off for 5. Cycle the key five or six times, and on the last cycle, try to start it.
If it starts, replace the pump, strainer and fuel filter.

I had that same problem with my dodge grand caravan and now with my ford windstar. If you have done normal maintenance like changinf spark plugs and wires, your best bet would be to check to see if your egr valve is clogged or staying open. Or you may have a clogged fuel filter. If all else fails check your catylic converter. In the end that solved my problem. I also think you may want to check to see if your IAC valve. If none of them are the problem check to see if your PCV hose from the upper intake valve is rotted. If it is it causes air to leak. although this would happen all the time not just when it's cold. Good luck.
