Use the same form you would have used if you had filed on time. You may be looking at a penalty for late filing even if you are due a refund. The Feds don't assess a penalty for late filing if you don't owe anything but some states do. OH levies a $500 penalty for late filing even when a refund is due.

Call the IRS and they will tell you what you need. They will even mail you the forms. Also, most libraries have that information. They have a notebook of different forms. When you find the one you need; you just make a copy of it. If it's during tax time, they will have boxes of the different forms. It just so happens this is tax time. Post Offices have some forms; maybe not as many as the library.
If this is for last years taxes just get in touch with the IRS and they will mail the papers to you.
