A lot came from literature, but most had some basis in the real world.
Peg-legs and hook for a hand come from the pirates becoming injured and needing a substitute - and the only things they had were what was readily available from the ship. Hooks maybe not so much - but wood replacements were probably common.
Parrots came mostly from literature. It's possible that birds did hang around because of the leftovers and some maybe even became pets. But the idea of a parrot came from Treasure Island's Long John Silver.
I've heard a few stories about the eye patch. One is that it was used to cover an injury. Another was that it helped out when using a spyglass to spot other ships. And I also heard that it was used so that when going from light to dark places often (like hold of ship, treasure cave or the like) they would switch the patch so that one eye was already adjusted to the darkness and would be easier to see.

It seems pirates died piece by piece, lose a leg here, Lose a hand there, until they finally disappeared altogether.
Our 'image' of what pirates used to look like seems to have come mostly from famous fictional pirates such as Long John Silver who had only one leg and a pet parrot, and Captain Hook who had his hand cut off by Peter Pan and attached a hook to his arm in place of his hand.

He was stolen by seafarers and found a parrot while on an island the parrot pecked out his eyes struggling to get free.

the legend came from treasure island the use of crutches has been represented through time. it still holds some truth if a pirate injured his leg the cook would be called upon to amputate. later a substitute 4 said leg would be used anything laying about the ship usually wood it would be bound to the body with leather straps. but prefer monkeyphils answer xx

the pirate had his leg bit off by a shark then his hand and so had a hook for his hand and wooden leg put on. shortly after a bird shat in his eye it was the first day of the hook and so he scratched his eye resulting in losing the eye. The parrot was purchased to keep other birds repeating this.

okay, this is no help but I have to share it with you. Our grand-daughter is 3 and she thinks that pirates say," Arrrrrrgh lady ! Take the plant. " We love that ! It's something we love so much that we never tell her any difference.
