To me it seems that she caught this fish, and looking at it, realized that it was old, and had fought hard to live as long as it had, but it didn't fight her at all. So it seems the fish was tired of fighting, and had decided it was his time to go. Upon realizing this, the woman didn't want to be the one that took this fish, because it had fought so hard before, and maybe if it gets a second chance, it will fight again. Reminds me of the beginning of The Old Man and the Sea.
