I think that was Tommy Herr that hit the sac fly.

I'm know Willie McGee was one of the runners that scored. I'm guessing it was Ozzie Smith that score the other run, but I can't remember exactly.

To complete the answer above, the runner on first faked tripping on second to get into a rundown, after the second run scored he was tagged out making the run count as the final out was made after the score and the out was not a force out. One of the most brilliant baserunning ploys of all time.

I'm guessing (actually I'm pretty sure it was) Stan "The Man" in a world series game in the 1940's. Ennis Slaughter was on second, tagged up after the catch and kept going!!!
