I would say that Madonna in James Bond was one of the worst cameos in history.

Madonna was a fencing instuctor or something in the Pierce Brosnan James Bond film Die Another Die.

Sean Connery at the end of Robin Hood: Prince Of Thieves. But it was so bad it's actually good. It's funny cos he's mean to be King Richard (i.e. the king of England) but he's Scottish?! So stupid (but then so much of the film is, especially the geography that anyone inside the UK knows is wrong but seemingly the filmmakers didn't) but I love it regardless.
Oh and I saw Brad Pitt and Matt Damon in Confessions Of A Dangerous Mind, doing cameos on a dating show (I didn't hear them say a thing and didn't watch the whole thing), that was pretty crap (but then I don't care for either bloke personally and don't get the appeal, although I think Damon is an okay actor, I don't like him personally after what he did to Minnie Driver).

O.J. Simpson was in the Naked Gun series, and not as a cameo, he was one of the stars.
As far as the worst cameo, I'd have to agree with John Wayne in The Greatest Story Ever Told.

It surely has to be John Wayne in 'The Greatest Story Ever told' where he came on at the end dressed as a Roman Centurion, looked at Jesus and said "For surely he was the son of God" in an American, cowboy-like drawl.

Too many to mention....and they stretch that far back it is hard to remember. However, perhaps Alfred Hitchock was the most famous in his film the 'Birds' how he never got attacked by the birds is a bloody miracle.
