About the only way to read someones mind is to ask them questions, based on their responses vocally and through body language could someone read someone elses mind. If there are other methods, you will probably never hear about them, or how they work.

Yep. Live with a person for years and years, spend lots of time learning him/her, be sensitive to all his/her needs and, in th end, you'll even know what he/she is thinking. It has been proven to exist millions of times by millions of people who tell each other: "You've just read my mind."

I don't think a pre-occupied mind can do this. If once we come to a state of "No Mind", we may be able to do that. It can also be done by reading body language and facial expressions. But, the depth is limited.

Yes you could.
Read through their words written or spoken. Evaluate their behavior and body languages.
In psychology the true value of what's inside the mind is manifested outwards no matter how it is concealed.
