Hymen is like a cover with hole in it.

Breaking means it is torn to make way for the penis or torn during physical activity like sports. It is still technically possible to have some unbroken parts attached to the side of the wall, if it is partially torn. But it is very rare.

No, but you can have a form of plastic surgey to create a hymen. In some cultures this is a common practice.

No. Once a hymen is broken it remains broken. There is a surgery to repair it if you choose to. It's called hymenoplasty. See link below. Hope this helps you.
