Billy Couture was the only NHL player to be banned for life for punching a referee. He was reinstated after a year, I believe. Maurice Richard was suspended for punching a linesman. Tom Lysiak was suspended 20 games for intentionally tripping a linesman in a game. This is all I can think of off the top of my head.

On March 17, 1955, at the Montreal Forum, It will forever be remembered as the Rocket Richard Riot.
the Montreal Canadiens where playing the Boston Bruins. The Rocket was skating down the wing and a Bruins defender slashed the Rocket over the head with his stick. the Rocket was furious that the ref did nothing, so the Rocket got back up and slashed the Bruins player over the back and a brawl brock out. the Rocket was fighting two Bruins at the time then the ref grabed the Rocket from behind and tied up his arm's. the Bruins player came up to the Rocket and puched him right in the face then the Rocket turned around and puched the ref.
When the Canadiens hosted the Detroit Red Wings on March 17, 1955, a day after Campbell suspended Richard, the air was thick with tension. It got even thicker when Campbell showed up at the Forum 10 minutes into the game with three lady friends. He took his usual seat. Before long a young man approached Campbell and offered to shake his hand. Campbell instead got a slap in the face. Another man approached with a tomato that he rapidly hurled at Campbell, who remained calm and seated through most of the abuse. When a homemade tear gas bomb sent noxious fumes into the air, Campbell and his lady friends finally made their way to safety. The damage, however, was done. The game was cut short and angry mobs flooded into the streets to smash store windows, topple cars and vent their anger towards the percieved injustice and mistreatment of their hero.
The Rocket Richard Riot was underway. A riot that some claim was the symbolic beginning of Quebec's nationalist movement and most will remember as a defining moment in Canadian history.

Maurice Richard, I believe was in 1955.
He punched a Ref in the face. The ref was holding him while another player (I can't remember the team) was punching Maurice Richard. When the Rocket finally got loose from the ref. he turned around and smacked him in the face, the ref fell unconscious on the ice.
This lead to his suspension and the riots that took place in Montreal.

Tom Lysiak was suspended 25 games for hitting an offical with his stick while a member of the Chicago Blackhawks - it was in the 80s during the "Oiler Years" - I think it was a linesman actually

In years past, there has been shoving, hitting, pokeing, slashing, and checking of referees. That is where the present rules protecting the ref, came from.

i think it was curtis joseph in the playoffs a few years back, he said he slipped and hit the ref but it looked like a punch, when he played for the leafs

I know that one ref was talking about Mario flinging a puck passed his head and when he asked Mario about it Lemieux said "I missed."
