Do they ever just throw up a coin to make a decision?

Throwing up is not good advice for a gastroenterologist. Not all the sounds they hear are pleasant. Although their focus is abdominal the noises are abominable. It is not by coincidence that the first part of name of their profession is gas. It is pretty hard to stomach. They get a belly full of this noxious by-product. This may be hard to digest, but after awhile they can see a silver stomach lining.

No they usually go with your gut feeling and would rather direct a colonostopy that a symphony.
