He had some sort of disease in the muscle tissue and the tissue had to be removed. That is why he limps--he is disabled. And why he is addicted to Vicodin.
This was addressed in an episode with his ex-wife. He was angry with her for making the decision to do the surgery while he was incapacitated.

it is addressed interior the season a million episode "3 memories". on a similar time as golf, domicile suffered an infarction in his leg. He went to the ER, however the way he behaved made the well-being care professional have faith he became a junkie finding for painkillers, so he wasn't taken heavily and sent away. He clinically determined himself some days later, yet via that ingredient it became too late because of the fact the muscle became ineffective. He has a Muscle Infarction Affliction...

What House says about the leg problem:
A patient: "What made you a cripple?"
House: "I had an infarction."
The patient: "A heart attack?"
House: "It's what happens when the blood flow is obstructed. If it's in the heart it's a heart attack. If it's in the lungs it's a pulmonary embolism. If it's in the brain it's a stroke. I had it in my thigh muscles."
The patient: "Wasn't there something they could do?"
House: "There was plenty they could do, if they made the right diagnosis, but the only symptom was pain. Not many people get to experience muscle death."
The patient: "Did you think you were dying?"
House: "I hoped I was dying."

I think that he was in some sort of motorcycle accident. You know it is kinda hard to get a straight answer on that. When that lawyer who was his ex-girlfriend, but is now married to some guy in a wheel chair was on, like last season, I think they talked about it and the motorcycle accident was mentioned. I often wounder about it myself, if that is the reason, but that is one of the many reasons why we watch the show though, it makes you think. I guess it also makes you laugh your pants off too. LOL!
If you really want to know I would check and see if you can find a fan page or a website for the show try this and see what you find. Good Luck! http://www.fox.com/house/

"His crankiness is commonly attributed to the chronic pain in his leg (the result of an infarction in one or more of the quadriceps muscles in his right thigh)." This is basically a stroke in your leg muscle.
