It's a figure of speech/simile, and means largely that they feel exhausted or tired.
It is an extreme exaggeration, but you'd wouldn't go so far to say you are completely dead.

Simply because they feel tired and need a rest badly and it definitely sounds better than saying, "I'm half alive!"
