You need to get a health card from the state of Oregon. You can do it on line and it costs about 10-20 dollars. I can't remember if you have to register what company you are working with. It has mostly to do with what temperature you can serve food at, etc.

If you work in a place that serves alcohol you need to get an OLCC card (Oregon Liquor Commission). That is usually sponsored by the company you go to work for and requires a Saturday sitting in a state office listening to a lecture on how to identify if a person is drunk and how do you handle them.
