Hard for me to answer without drawing pictures, but here's a try.
First,- it is a cousin once removed and refers to relationships. Here is a try:
I am A. I have a first cousin B (we are first cousins because our parents are/were siblings.)
I have two children C and D, My cousin (B) has children E and F. The four children are second cousins to each other because they are the offspring of cousins.

I (A) am a first cousin once removed to E and F, because their parent and I are cousins, not siblings.

This relationship is often erroneously called second cousins, but as you can see, this is not correct. Hope this is understandable.

A first cousin once removed is the child of your first cousin. The child of your first cousin once removed is your second cousin.

Lets say my mothers sister is my aunt and my aunts child and me are first cousins(born of two sisters) Now my cousin has a child and I have a child so they are cousins once removed. alittle complicated at first i guess.Then the next set of children from those cousins would be third cousins. bettyk

It refers to the generations removed from you.

You and your Mother's Sister's Child are first cousins
Your cousin's child is your first cousin once removed (one generation removed from you).
Your child and your cousins child are second cousins.

You + Your cousin = First cousins
You + Your cousin's kid = First cousins, once removed
You + Your cousin's Kid's kid = first cousins, twice removed
Your kids + Your cousin's kids = second cousins
