Wealth Inequality, no doubt - consider that as the world's population grows, billions of people lack the proper food, water, shelter, and basic tenets of life, whereas a relatively very few of the CEO-type businessmen control the majority of the wealth, power, and resources.
Genocide and Ethnic Violence: places like Darfur, the Iraq conflict, the Palestinian-Israeli issue, the Japanese government's denial of the submission and rape of thousands of Chinese women, etc.
Those two issues would certainly top my list.

Global warming and the end of fossil energy.
The combination will be deadly for possibly billions. While warming will cause hundreds of millions to move inland, and north, the lack of fossil energy will result in lack of fertilizers and transportation for food and clean water. Much of the world's population will starve to death.
