I can only find mention of one - a 14 year old boy named Alan Fish, who was hit by a foul ball at Dodger Stadium on May 16, 1970. The link below also notes that there have been 4 other fans killed in the minors (though not all by foul balls).

Hello Al and Jeremy, 5 people have been killed by foul balls since MLB's inception. If this includes the minors, you both may be right. It's also a trivia question, but I'm not sure to which trivia game.

An artical at the pennstate library confirmed at least thirty-five fatalities since 1900 in Major and Minor League ballparks. Interestingly only five are ball related, and only two of these are from being struck by a foul ball. The remaining thirty deaths are due to either fan behavior or stadium infrastructure.
