I am sure Umaga and Rikishi knew about it before it happened. General rule of thumb: If you are going to talk anybody in your skit, you have to talk to them before, tell them what you are going to say and honor their request if something is taboo.
You mean you don't poke fun at family members. We do that to each other all the time. Especially at huge family dinners where to poke fun at each other (politely mind you) in front of everyone. Maybe the Rock and his family is the same way.

That's part of his gimmick. Making fun of others while being "untouchable". The Rock is just great at improvising.
