I recently got married. Now it's time to change my last name; but I don't know how, or where to go, or what kind of legal documents I'd need to bring along. I need a new Social Security card and a new driver's license. Is there some kind of Social Security office, or place like that, where I can go to fill out some forms or something? I have already searched online, but found only forms to buy--'online'; which is rather difficult to do when you don't have a credit card. I live in (and was born in) Missouri, if that makes any difference. Someone, please help me, I am so lost.

You need to go to the Social Security office. They will tell you to bring your marriage license, and old social security card. THat is all you need to change your name on your social security card. THen at the dmv. you might need your ss card, drivers licenses, birth certificate.

Check online for the social security office nearest you and go there and tell them you need to change your name. (don't forget to take along your birth certificate showing your maiden name as proof of who you are). Go to your local DMV and fill out a change of address. I don't know where you have looked online but Social Security does NOT offer their forms online you have to GO to an SS office to make any changes. I would have to say that wherever you looked online that is charging for the forms is bogus. You will also have to inform all of your credit card companies of the name change and whether you want your spouse to be on your cards or not. If you are employed you will have to change your W4 with HR for tax purposes plus change your name on your work file. You will have to change your name on any health/life insurances as well as change your name with your doctor(s). But your best bet is to do them in person so that there are no mistakes made. I also suggest you go to the Social Security office first because your driver's license is connected to your social security number and if you change your driver's lisence before you get a chance to change your social secuirty and get stopped for any reason you can get into trouble for "identity theft" and will have to go through a bunch of crap just to prove you are who you are...So first SS then DL then HR then the rest.

you don't really need a new social security card, but you can take care of everything at the local courthouse and DMV. make sure you have 2 rescent letters or bills or something addressed to you (within the last 6 months) and your drivers license, social security card, birth certificate and then go to the courthouse first i would assume. also maybe bring your marriage certificate or lisence if you have one and make sure you include that you just got married to your hubby so that you're changing it for marriage purposes etc

go to the DMV with your marriage license and yes there is a social security office you just need to find one close to you. and if your name changed you do need a new social security card showing your new name. but i would get your drivers license first then go to the SSI office and change your card.

I don't know if it's like this in every state, but in PA the social security card has to be changed before you can change your license.

And dont feel bad, I got married in November, and switched my SS card, but have yet to switch my license OR my credit cards lol. I hardly drive, so I figure whats the point?! :)

All you need is your marriage certificate. Go to the social security office..(usually at least one in the county you live in)..It doesnt cost anything at the S.S. office..then go to the driver's license place and renew your I.D. show them your marriage certificate and new S.S. (or where you applied for a new card)

Here is the web address for the Missouri Vital Statistics Bureau/Office- they list phone numbers for contacting and change of vital stats records. Also listed are the addresses of where to go, and while contacting them, you can find out specifically what forms of I,D. that you need specifally.

Best of Luck to you!!
