Here's a quick summary of the story.

Perfection. That's all Damian cares about. But what happens when the urge for perfection goes too far?

It's basically about a boy with an eating disorder.

The Clock Struck Lunch, The perfect recipe, Pie of Perfection, Damien Binge, something like that. Hungary Again, Recipe for Perfection is a good one.

Hmm...kind of hard to say with just that. It could be a line in the book that says a lot or it could be just what it's basically about.
Perfection to Destruction maybe?

or include a metaphor in the book that you can turn into the title. (i.e., Of Mice and Men, To Kill A Mockingbird)

Just "Perfect" or "Perfection" might be good.

Or, you could try "Hollow", "Destructive Habits" or "His Little Secret".
