I focus on the music, the minister's voice and words.
I join in singing/praising.
I pray and listen to prayer.
I listen to and read the message being presented.
I concentrate on the purpose of the service.
Why don't you worry less about what you're guilty of and more about loving God, loving your neighbors as you do yourself and sharing/spreading the word with others?

I might think about what someone is wearing, but not in a mean way. Usually only god, though. If i was there for othber reasons, then i wouldnt go
