In order to learn from a mistake, first we have to actually acknowledge that we've messed up! And then... from acknowledgment comes accepting responsibility for our actions. After that... well... we have to sit down and actually think and analyze what we will do differently next time, should the situation arise again.
Learning can be a rather steep curve... if any part of the above is ignored, I don't think that learning truly happens. And of course, there is always fear and anxiety factored in there at any step of the process...
