Take several sheets of paper and head each sheet with each individual mental state that you are storing. Below each heading list all the subheadings that are associated with the main category. Soon you will have all your thoughts visually displayed. Separate the sheets on a table or floor then prioritize each in order of importance or relevance to a certain goal. Collect the sheets in order and put them into a folder. The folder will be there for future reference, re-sorting, or an addition to the set upon a new mental state. Sheets that are totally unimportant and display all that you no longer want to think about you can crumple up and throw away. Then you wont be wasting anymore of your mental energy thinking about them.

Do not read any psychology self help books, they will screw you up even more.
Drink lots of water, eat healthily with lots of fruit and veggies, Avoid alcohol and drugs, do some regular exercise and adopt a kind and gentle attitude to those you love. Try to avoid confrontational situations and do not be so introspective, psychology is not a very mentally healthy thing to get involved in.
If you are looking for some great psychological exercise to "train" your brain into "perfection" do some meditation, it will rearrange it all for you.
I think you should not wait for mental perfection to drop out of the sky before you act, you will never get anything done if you wait for that.
Do not ignore your body, it is much more than something to get your mind from place to place.

I like lists. They are tangible and you can't forget what you have written. PROS - CONS - ACHIEVEMENTS - WHAT I WANT TO ACHIEVE. To sort yourself out go over the cons and sort the ones you can, if there is nothing you can do about some of them put them aside. Pros - work on making that list longer. Achievements - Be proud of the ones you have reached and intend adding to them from the What I want to Achieve list. It sorts your brain out, gets your mind focused and enables you to sort out the cons that have been niggling at you so that you have your present state in order. Then you can move on.

No lie, do this:
Take a broom. Sweep the sidewalk.
Walk around the block. Just walk.
When it is raining, stand in the rain.
When it is cold, watch your breath form fog.
When you are hungry, eat food. When you are tired, sleep.
Get some books on Buddhist Koans, and study them.
Good luck.
